Care Management

Care Management is the coordination and supervision of a wide range of in-home services to maximize the well-being and comfort of clients. Our care managers begin with a thorough assessment of each client’s physical, social and emotional needs. We plan for meeting nutrition needs, safety and cognitive functioning as well as health care and medical requirements. Working with the client and the family, we then develop and monitor a care plan that evolves as levels of care change.

Équinoxe Care Managers are skilled nurses or social workers, with recognized credentials in gerontology, home care and nursing. Their expertise enables them to structure the most appropriate support for short or long-term care. Families can feel secure that their loved ones' interests are completely protected. Équinoxe Care Managers act as both consultants and advocates. Maintaining client confidentiality is of the utmost importance.

Équinoxe is a member of the National Association of Professional Geriatric Care Managers (NAPGCM) and our caregivers follow the association’s standards of practice.
